The Third Trimester: Getting Closer to Your Journey’s End

The end is near and you begin counting the days to your due date. Again, fear does not go away since giving birth is another event to overcome on your way to motherhood. Like any other endings, the third trimester is a harbinger to a more exciting event in your life. With the remarkable developments in the last few weeks as pregnant mother, the last trimester is both an ending and a beginning – the end of the baby’s life inside your womb and the beginning of his journey outside as an individual. As you prepare for all baby materials for the coming of your child, your baby also gears up for getting out of the womb. How soon can I begin a diet and exercise after I have my baby, since I plan to breastfeed for 6 months?

I would like to begin dieting (safely) as soon as possible after my daughter is born.
I am wondering if my diet and exercise will have a negative effect on me or my newborn, since I have been told that I should be consuming more calories while breastfeeding than if I wasn’t.

I was told to wait about 2 months until I started dieting to assure that you build an adequate milk supply that wont be affected once you lower your calorie intake. As long as you are eating a nutritious foods you will be fine. I think you burn about 500 calories a day breastfeeding so that will make up for the extra calories you have to consume to breastfeed. You need to make sure that you eat around 1800 calories a day or more while breastfeeding. Most moms can safely lose up to 1.5 pounds per week or 6 pounds per month after the second month and not affect milk supply or baby’s well being. Also when you start to diet decrease your calorie intake slowly. Don’t just make a sudden drop since this can affect your milk supply. For me I feel like breastfeeding actually helped tone the muscles in my abs.

 Parents want to protect their children when surprising circumstances arise.  Whether utility service has been interrupted or you’ve been unexpectedly stranded, there are ways to be prepared.   PREPARED, NOT PANICKED  Panic is a great danger in an emergency – as mentioned in the article: RULE OF 3′S TO SURVIVE.  Children will pick up on your emotion.  Your preparation will make you confident so that your children will draw strength from you.   THE NEED FOR A ROUTINE  Look for creative ways to implement a daily routine for the sake of your children.   A parent instituted routine is comforting to the child.  PASTIMES   Amusements help pass the time when you are unable to travel.   It is unlikely that you will have access to a source of power to operate electronic games.   Craft supplies are wonderful to have on hand; this includes play dough, scissors, paper, tape, markers, glue and other assorted items.   Take the time to introduce your children to board games, dolls, toy cars, blocks, puzzles and reading in order that they will be welcomed ‘old friends’ should they need entertainment.   It is also wise to keep educational materials stocked (items such as work books, flash cards, pencils, reference books, etc.) in the event you find organized schooling is no longer an option.   MAINTAIN YOUR HEALTH  Your dental health and other health conditions should be tended while you have access to remedies and treatments.  Rather than neglect those annoying aches and pains, take steps to remedy them so they aren’t a factor in an emergency.  Remember to keep any necessary medicines in stock.  Get into the habit of observing the sleeping, eating and toilet patterns of your family members so that you will detect illness early and be able to deal with it proactively.  Train yourself to take note of the impact of circumstances on those around you. Contrary to advertisements, babies can thrive without cribs, bouncy chairs, carriers, disposable diapers, pacifiers, music boxes, and other assorted gadgets.   An infant’s needs are simply to be fed, warm, dry and clean.  Nursing an infant is a skill that can be learned.  Even if you should choose to give your infant bottled formula, it is in mother and baby’s best interest to invest the time to breastfeed during those crucial first months.   This can help insure that the smallest family member has a ready supply of nourishment.   For bottle-fed babies, you will need to keep formula in stock.   Infants require protection from extremes of cold and heat as their bodies aren’t yet able to regulate temperature.   Babies need to be shielded from the summer sun.   Umbrellas are useful for this application.  In the winter, dressing little ones in layers protects them from the cold.  Zippered fleece sleepers can be placed over long thermal underwear for nighttime warmth.  Sharing a bed with 2 or 3 others helps everyone stay warm at night.   Wool socks, hats and zippered fleece jackets can keep babies warm during the day.   SANITATION   Daily baths won’t be possible if water is in short supply or if temperatures are very cool. Give your children the gift of learning how to do such things as shop wisely, stock a pantry, pitch a tent, build a camp fire, cook, sew, garden, repair small engines, perform first aid, CPR, and swim.   Understanding what to do during a fire drill, how to fend off an attack and having practiced other scenarios are real confidence builders.   Prepared children aren’t as likely to fall apart emotionally.   When preparedness is made part of your daily living, your children will be better equipped to deal with emergencies.  Rather than face the unknown with desperation, you will have a plan and your little ones will know what to expect.   Strengthen your family today with these measures.  Unexpected circumstances present parents with challenges in caring for their children. No matter whether you’ve been stranded or if your electric service has been interrupted, planning ahead helps you deal with the unexpected.

Help! I need to lose 50 pounds, but I’m injured and can’t exercise. Any advice on how I can lose the weight?

I need to lose the 50 pounds of weight that I packed on during my pregnancy to get to my goal weight of 120 pounds. I have 2 problems. I’m breastfeeding so I can’t restrict my calorie intake too much and I’m also injured. I have 2 herniated disks in my lower back and because of this I have a really horrible case of sciatica. To sabotage my efforts even further I have a debilitating case of endometriosis that leaves me doubled over in pain on an almost daily basis. So basically I’m in pain from my lower back down almost all of the time and I’m exhausted from being a new mom. How can I drop this weight without working out? Is there some weight training that I could do with my arms or something? I have no idea how to handle this so any advice would be wonderful.

You don’t need to work out to lose weight. It just helps the process along. Even a mild execrise like walking will help.

Also, the breastfeeding helps you lose weight. Milk production will require alot of calories and you have alot of extra energy stored in the left over pregnancy weight so dont worry too much about a calorie reduced diet. Just take your vitamins , drink a good amount of water and eat healthy. You will notice the weight drop.

I know that breast is best…that it has more nutrients etc…I also know that breast feeding promotes a close bond …but Mothers who bottle feed also feel a close bond whilst feeding…why do they say that bottles and teats are a bad habit after one…but breasts aren’t? I just answered a question about breastfeeding in public…I think it is fine but I did think “Oh gosh!” when I saw a girl the other day sitting on a bench on a busy street with her boob hanging out and a baby latching on it….do girls not worry about perverts staring at them or makng weird comments and stuff? I would…there are loads of pervy men about especially in cities and towns…I think that is why a lot of people are uncomfortable about the whole feeding in public thing….I think women should be free to do it but I know I would worry about weirdos too much.
So the question is..do you worry about perverts..or are you not worried about them? Have you ever been stared at by men whilst feeding and what did you do about it?
I know there are lots of girls in scanty clothes…but it is not the same a s a bare breast….I am not judging…I think it is beautiful…but men can be weird.

Pervy guys are going to stare and make comments regardless of what you are doing. Most pervs are not interested in looking at a boob when there is a baby attached to it anyway.

Easy Food Idea’s for Breastfeeding – NON Dairy!?

I am breastfeeding my 3 month old. She had bad bad gas int first month she was born. I was changing my diet so much.. thinking it was this and that… then i read dairy can raelly give a baby gas. Our Dr. advised yes its true. Give it 3 weeks with NO dairy to clean out your system and then slowly start with 1 ounce a day….

Well since then I have had NONE! I have watched and checked every label to insure there was nothing dairy in anything… bread, cookies ect…

I have still not done dairy… i sometimes will eat some bread or a bite of a dessert that i know is loaded with dairy. And she has a gassy night :(

I LOVE my Trader Joes *Unsweetened* Soy Milk… Soy Cheese….

So please send all receipes or fast foods I can make to eat that are dairy free or I can use my soymilk with…

The easiest thing I make is grilled/broiled skinless boneless chicken breast, and then I cook up some mixed peppers and onions and eat with it. You could even add the soy cheese ontop of the chicken.

Malaria in Africa summit opens in Nairobi

A CALL for substantial and sustained support for research to guide evidence-based policies and development of new malaria tools, to save countless lives, sign-posted the world’s largest malaria conference, which opened in Nairobi, Kenya, yesterday.

And pneumonia, one of the biggest causes of child death in the world, will by 2011 be controllable with the use of a Pneumonia Conjugate Vaccine (PVC) scheduled to be available for routine immunisation of children below five years.

The World Health Organisation (WHO) and the United Nations Children’s Fund (UNICEF) will also invest $39 billion to tackle the disease.

The vaccine, being developed to reduce the incidences of newborn and child mortality, is to help in the attainment of the Millennium Development Goals (MDGs) 4.

The 5th Multilateral Initiative on Malaria (MIM) Pan-African Conference brings together 2,000 researchers, health workers, public health officials, policy-makers and activists from across Africa and the world.

According to a Pan-African News Agency (PANA) report, Kenya’s Vice President, Kalonzo Musyoka, opened the conference, the first to be held in four years. It will highlight the latest research in malaria prevention and treatment.

The theme of this year’s conference is “Building knowledge for action”, but the key question for malaria is which knowledge for which action?” said Dr. Adrian Luty, Senior Researcher at Radboud University Nijmegen Medical Centre, The Netherlands, and Chairman of the MIM conference Scientific Committee.

“If we as a global community are to be ready to achieve eradication, we must invest in the science that can guide these efforts and provide the tools and methods that will make success possible,” Luty said.

Although preventable and treatable, malaria continues to be one of the world’s most deadly and persistent diseases.

Ninety per cent of all malaria deaths occur in Africa, and 85 per cent of these deaths are among children under the age of five.

Since the last MIM conference in 2005, the global community has made tremendous strides against the disease, the conference organisers said.

But they noted that despite this progress, many African countries are struggling to meet Millennium Development Goal 6, which aims ‘to halt and begin to reverse the incidence of malaria,’ and are unlikely to meet the WHO target to halve malaria mortality by 2010.

One of the key goals of MIM is to strengthen African research capacity to guide policies for malaria control and to develop new tools for prevention and treatment.

Research conducted in Africa has helped guide new practices, for example, in Rwanda, where just five years ago malaria was the leading cause of death.

A multi-faceted approach using long-lasting insecticide treated nets (LLINs) and training for community health workers has decreased the malaria death rate by 60 per cent in just two years.

In Zanzibar, a mass distribution of LLINs in early 2006 and insecticide spraying programmes triggered a 95 per cent reduction in malaria infections for children less than two years of age.

“Over the past four years, new strategies coupled with unprecedented global attention and support from endemic country governments have increased access to malaria treatment and prevention for some of the world’s most vulnerable communities,” said Dr. Francine Ntoumi, MIM Secretariat Co-ordinator at the African Malaria Network Trust (AMANET), which has hosted the MIM secretariat for the past four years as the organisation’s first African secretariat.

“However, we must continue to build a critical mass of scientists working on malaria in sub-Saharan Africa, so that countries with the greatest burden are themselves setting the research agenda,” she said.

Due to continued cost and availability challenges, those most at-risk of malaria often have limited access to proven prevention tools, such as insecticide-treated nets and access to the current gold-standard drug, artemisinin-based combination therapies (ACTs).

As a result, the disease disproportionately affects poor rural communities in malaria-endemic countries.

In Nigeria, the Minister of State for Health, Dr. Aliyu Idi Hong, at the World Pneumonia Day celebration yesterday in Abuja, stated that although “the vaccine is not in the market all over the world as we speak, it is still under production, but I’m confident that it will be available by then (2011)”.

The first of its kind, the celebration with the theme, “Prevent, Protect and Treat,” is aimed at drawing attention to the disease as a public health concern and increase awareness on its prevention and control.

Pneumonia is the second killer disease of children under five in Nigeria after malaria, with about six million new cases yearly. It kills about 200,000 children in Nigeria and two million globally.

A joint press statement by the WHO and UNICEF spoke of a strategy called the Global Action Plan for the prevention and control of Pneumonia (GAPP), which aims to save up to 3.5 million children from dying by 2015.

According to the two bodies, the cost of implementing the GAPP by scaling up the recommended measures in the 68 high burden countries is estimated at $39 billion between 2010 and 2015. The cost is expected to rise over the six-year period from a yearly need of $3.8 billion in 2010 to $8.0 billion in 2015.

The Director-General of WHO, Dr. Margaret Chan, said: “This action plan provides the strategy to prevent and control pneumonia, which today kills more children than any other illness” and that “we know the strategy will work, and if it is applied in every high burden country, we will be able to prevent millions of deaths.”

UNICEF’s Executive Director, Ann Veneman, who also stated that the disease, a leading cause of death of over 4,000 children daily, added: “Effective interventions to reduce deaths caused by pneumonia must be used more widely and made more readily available for children at risk.”

Caused by a bacterial, viral or fungus infection, prevention of the disease was identified as a key to solving the problem through exclusive breastfeeding, proper nutrition for older children, reduction of indoor air pollution from tobacco and stove smoke as well as improvement of pre-natal care to reduce incidences of low birth weight.

To this end, however, other underlying diseases like measles and diarrhea should also be prevented from gaining grounds among children.

Colic, Breastfeeding and Infant Nutrition – Fat is Where Its At

Colic- what a word!  This little five letter words strikes fear in the hearts of most new parents and brings to mind visions of sleepless nights, and endless frustration in trying to parent the new bundle of joy.  Any parent who has ever had a colicky baby would not wish the same fate on anyone.  Some people think that colic only happens to formula fed babies but this is not so. Breastfeeding is not a guarantee that your baby will be free of infant colic.

What is infant colic?

Colic is simply a word that means stomach pain.  In the absence of any real scientific evidence the widely held belief is that infant colic results from intestinal cramping.   Estimates are that colic occurs in about 20-30% of all babies at some time or another, most commonly in the first three months of life.

How will I know if my baby has colic?

Infant colic is easily identified by the baby’s behaviour especially after feeding. The baby acts like she is in pain.  Crying is often accompanied by vigorous leg movements and the child may spasmodically extend her legs during a bout of crying.  Sometimes she will expel gas and have green stools.  The baby’s crying escalates as the discomfort increases and parents and babies alike become fretful.  Nothing is more distressing that a baby who cannot be consoled.

Can a breastfed baby develop infant colic?

There can be no argument that the best source of infant nutrition is your own breastmilk, but even if you are breasfeeding, colic can still occur. There is absolutely no reason why breastfeeding should be discontinued if your baby is showing signs of infant colic.  There are, however, things that you can do to change your  breastfeeding technique and reduce the possibility and severity of the colic.

Breastfed colicky babies  are very often good eaters, and have a good healthy pattern of weight gain.  Parents often say that the baby seems hungry all the time but the more she is fed the crankier she gets.  Some parents misinterpret this to mean the baby is not getting enough breastmilk and in desperation may switch to formula thinking that mom does not have enough milk.  This is absolutely not the case.

In fact the breastfed baby who is experiencing infant colic is probably getting too much breastmilk, and too much of the wrong kind of breastmilk.

Here’s why:

When you put your baby to the breast the baby starts to suck and a hormone is released that makes the milk ducts contract and breastmilk is literally “squirted” into the baby’s mouth.  This is called the let down reflex and you might notice it as a slight aching that occurs within the first few minutes of the baby on the breast.

During breastfeeding the fat in the breastmilk clings to the sides of the milk ducts and is not released until the end of the feeding.  Babies who have short feedings are often getting lots of the skim breastmilk (called “foremilk”) and not the fat rich milk at the end of the feeding (called “hindmilk”).  Because of the lack of fat hunger returns quickly.

Your colicky  baby is actually getting lots of milk but the breastmilk does not have enough fat so she is not satisfied for very long.

The foremilk  your baby is receiving has a high concentration of lactose and can cause intestinal cramping.  Lactose is such a powerful stimulator of intestinal contractions that it is used in high concentrations for the treatment of adult constipation!  Is it any wonder that the breastfed baby who gets lots of lactose in her feedings has intestinal cramping and diarrhea stools?

As a breastfeeding mom what can I do to help my colicky baby?

Your main objective will be to make sure that your baby gets more of the fat rich hindmilk.  This reduces the concentration of the lactose and gives the baby the fat needed to create satiety.  In other words the baby will be satisfied for longer if she gets more fat at each meal.

  • express milk before the feeding.  This starts the flow of milk and reduces the amount of foremilk that the baby receives.
  • nurse only from one breast at each feeding. This makes sure that  the baby completely empties the one breast and gets all of the fat rich hindmilk that lingers in the milk ducts until the end of the feeding.
  • sit your baby up.  This may require changing your usual position and using the “football hold” . In the football hold the baby is held by your side facing the breast that the baby will feed from.  If you are nursing on the right side, place the baby beside you, on your right side facing the right breast.  Your right arm should go around the baby , along her back and hold her at the back of the head.  You will need a soft pillow to create a little “seat” for the baby so that she is more upright than lying down.

If these measures do not alleviate infant colic then you could try eliminating cow’s milk from your own diet.  There is lots of good anecdotal evidence to suggest that some infants show signs of improvement when mother eliminates dairy products.  However, try the other measures first as they are more likely to bring about the best benefits for you and your baby.

You can and should continue breastfeeding your colicky baby.  There is no reason to withhold the best possible source of infant nutrition from your baby if she has infant colic.  Now that you know what causes infant colic, a few simple changes in your breastfeeding technique can make all the difference.

Did you have difficulty establishing Breastfeeding? Would you have benefitted from the ‘mythical’ Lactation Consultants? Did you see an LC? Do you think the NHS, who make such a big deal of BF, should train and employ BF specialists?

Personally, my closect LC was 140miles away (not exactly convenient for a home visit). I had great difficultys with my first daughter, I was basically told to work through them on my own – the midwifes answer to a poor latch was forcing my babys head on my breast, it was irrelevant that she was holding her so close in that she couldnt breathe.

I had problems for a long time. i had help in the unit where i gave birth, where i was lucky to stay for four days, but one of the midwives left me distraught one night after telling me i was just going to have to bottle feed, as some people just can’t breastfeed! this was after forcing me to pump for two hours, telling me i wasn’t producing enough milk for the baby, despite the fact it was only a couple of days since the birth and my milk hadn’t come in yet! eventually, after three weeks of sitting permanently on the sofa i managed to get baby to latch on – i wasn’t going to let that midwife be proved right! I heard the term lactation consultant, i even thought about trying to contact one, but had no idea where to start. my hv was fab and popped by the day i came home with the baby – after hearing me sobbing down the phone that i couldn’t feed my baby and she was starving and i didn’t know what to do! she was comforting, and tried to help, and assured me i was doing well, but i just don’t think she had the answers that a lactation consultant might have! Also half of my antenatal friends,all of whom wanted to breastfeed, didn’t manage to. Most gave up before they even left hospital, others when they were told they weren’t producing enough milk. I just think it is such a shame they couldn’t get the professional hel p that would’ve helped them continue!

Hi, anyone have a healthy diet plan for a breastfeeding mom?

I have a very colicy baby. I need something like a breakfast/lunch/dinner and snacks in between type menu. I don’t need answers that just tell me what I can eat and what to avoid. Ideally, 2000 calories or less and no milk/dairy products, nuts, caffeine, things that can make a fussy baby even more fussy. Anyone following anything like that?

You should add an additional 500 calories to your diet, so somewhere around 2500 calories, but not much more.

I try to avoid all beans, cauliflower, broccoli, brussel sprouts as these can make the baby gassy. I also avoid caffeine, but I still eat dairy products (I would avoid those only if your doctor suggests so from the baby seeming to be lactose intolerant, such as redness or vomiting). I also drink a lot of water.

These are my suggestions and have worked very well to keep my baby calm and happy.
